Alumni reading cards are being issued by the Library Information Office, located in 201 Butler Library. Please visit their website for more information.

Retired faculty and staff may receive a University ID Card as long as the retired status can be verified in DIA. If we are not able to confirm your retired status, please contact your Department Administrator.


Please call the CUIMC ID Centers at 212.305.0238 or Manhattanville ID Center as soon as you realize you lost your card. After business hours, please call CUIMC Public Safety at 212.305.8100 or Manhattanville Public Safety at 212-853-3301. A replacement card can be issued for a $20 fee.


If you submit a copy of a police report, we will replace your ID Card at no charge.

 


A new picture can be taken and a new ID issued for a $20 card replacement fee. You will be given two chances at a new photo. The fee will be waived only if: your appearance has changed drastically since the original picture was taken; the original picture is of exceptionally poor quality; or a security officer stopped you because the image is faded.

 


If you have changed your name, please ask your Department Administrator to update your information in DIA (Delegated Identity Administration). If the name change is not appearing in DIA, bring your old ID showing your previous name and a copy of your marriage certificate, social security card, or other government identification which verifies your new name.

 


Ask your new Department Administrator to add you to their population in DIA (Delegated Identity Administration). Once the change can be verified in DIA we will print you a new ID. You must turn in your old ID before you can receive a new one.

 


Any card that has been bent, broken, hole-punched, sat on, or that shows any noticeable signs of neglect or damage, will be replaced for a $20 fee. Please do not keep your card in your wallet in your back pocket. Cards that appear to otherwise be in good condition will be replaced once and we will waive the fee. Any subsequent replacements for damaged cards, regardless of its outward appearance, will be charged the $20 fee.

 


The Columbia University ID card serves as proof of student/employee status within the university. Students and employees are require to wear a visible CU/CUMC ID card while on CUMC and NYP properties at all times.
